Medical Transportation Information

Nonemergency medical transportation services are available for Medicaid beneficiaries and their children, ensuring easy access to your therapy appointments.

Call in Advance:

• Local Appointments: Call at least two workdays before your appointment.
• Out-of-County Appointments: Call five days in advance.

Phone Hours:

• Phones are answered Monday through Friday, from 8 a.m. to 5 p.m. local time.

• Children 14 and Younger: Must be accompanied by a parent or guardian.
• Children 15 to 17: May travel alone but must have written permission from a parent before scheduling the trip.

• Medicaid ID or Social Security Number of the person with the appointment
• Name of the medical professional you or your child will be seeing
• Address and phone number of your appointment location
• Address and phone number of your pickup location
• Reason for the visit
• Date and time of the appointment
• Special needs (e.g., wheelchair, lift, walker) to ensure the appropriate vehicle is sent
• Confirmation that other means of transportation are unavailable

If you have a car but no money for gas, call us using the steps outlined above. Inform the representative that you need help with gas money. They will mail you an application form to become an Individual Transportation Participant (ITP).

Medicaid can also pay someone else, like a relative, friend, or neighbor, to drive you to your appointments if they sign up to become an ITP.

• Current inspection sticker
• Valid car insurance
